Elderly
Homemaker Service, under the umbrella of Combined
Community Action was developed in 1978, to assist
impaired elderly (60+) to remain at home and be
independent, as well as enhance the quality of their
life and social well being.
Case
of a typical Homemaker Client
- 85
year old widow - Lives alone
- No
immediate family in the area
- Falls
and fractures hip
- Hospice discharge
- Cannot return home without assistance
- Homemaker needs assessment and goal plan setup
- Homemaker service approved for 3 days a week, two
hours each visit
She is
able to remain in her own home, living independently and
secure in the knowing she has help with the things she
cannot do for herself.
Homemaker Goal
The
primary goal of the Elderly Homemaker program is to
prevent premature institutionalization for the at risk
frail (60+) elderly, by providing advocacy and
assistance.
By the
aide observing and reporting changes in the physical
and/or emotional status of the elderly client,
unnecessary progression of illness, disease and
deterioration can be prevented.
The
clients overall nutritional status will be maintained,
and hopefully, improved. This will prevent skipping
meals due to the inability to prepare them.
If the
Program Director fees that the physical and/or emotional
well being of the elderly client is in jeopardy, linkage
and referrals are made to proper resources.
The
overall quality of life is enhanced.
